In-Memoriam Photo At The Oscars Featured Living Woman

Leading Australian producer, Jan Chapman is still alive but apparently someone at the Oscars didn't know that. She was featured during the In Memoriam segment and Jan was said to be devastated when she saw her face on the screen.
